Us Weekly reports that Warner, the former star of Bravo‘s Work Out, was arrested Friday, Feb. 24 and charged with assault with a deadly weapon, driving under the influence and hit and run. Police told the site that she was arrested around 5:45 p.m.

“She was arrested and the investigation is ongoing, and the case will be presented to the district attorney’s office upon completion of the investigation,” a detective with the West Hollywood Police Department told Us Weekly. “She is not in custody at this time.”

TMZ reported more details on the incident, saying that Warner crashed her car into a pole. The site also says that when law enforcement arrived to the scene of the crash, Warner backed her car into a police car, nearly hitting a deputy in the process.

Sources close to Warner told TMZ that she wasn’t drunk when the incident occurred — they say she went to lunch and only had a single martini, but then went home and took an Ambien. According to friends, that’s the last thing Warner remembers — when she woke up, she was in the hospital and had no memory of the crash or her arrest. Friends told TMZ she was “sleep driving,” and that when she was taken into custody, she wasn’t wearing pants.

Warner, who worked as a fitness guru on her Bravo show, admitted to partying amid a sort of breakdown when she turned 40.

“I didn’t like doing the type of TV that I was doing which made me uncomfortable and uneasy,” she wrote on her website at the time. “I had succeeded in many ways but always had very tumultuous relationship, which caused me quite a lot of misery. Shortly thereafter, I sold my clinic and gym and found myself wondering what’s next?… I felt a bit lost and started partying a lot. It felt like I was taking a few steps backwards in life and I started making bad decisions.”
